Location: Based in Narre Warren but will oversee our SIL properties in Victoria

About Us:

We Care Staffing is a leading Disability Service Provider specializing in providing Allied Health Care Workers such as Disability Support Workers and Nursing staff to many of health care organizations in Victoria, ACT, NSW and QLD.

The SIL Coordinator will be responsible for:

Individualized, professional customer intake
Assessment experience for all new customers transitioning into WCSS Supported Independent Living (SIL) services.
Ensure our customers experience a seamless and positive transition into WCSS SIL services.
Engaging with WCSS customers, service operations, WCSS staff, key decision makers, external providers, and funding bodies
Ensuring adequate information is gathered regarding the individuals’ immediate support requirement.

Develop support plans for each individual once accommodation commences by:

Undertaking detailed assessment of everyone to ensure the “best fit” in the development of support plans to
Address their personal, health and skill development needs.
Developing documentation procedures for staff and review processes to ensure the ongoing monitoring and evaluation of outcomes
Communicating with staff the objectives and strategies for each support plan and the documentation required.
Ensuring strength-based approaches and least restrictive practices are an integral part of everyone’s support plan regarding the provision of daily living support

Skills & Experience

Experience in case management and stakeholder engagement
Excellent communication skills
Experience in NDIS Roster of Care (ROC)
Sound knowledge, skills, and experience of the disability services sector
Well-developed understanding of the NDIS
Strong administrative skills and proficient in Microsoft Office
Build and nurture relationships with customers, families, and your team by encouraging open communication and participation
Identify, assess, and resolve problems with a customer-centered approach
Budget, prepare and monitor financial systems in line with the NDIS price guide
Support customers with mental health and high physical support needs
Practice leadership within team meetings, one on ones, modelling, and coaching with staff
Proactively train staff to ensure they continue to provide the highest standard of customer outcomes
Willingness to take on ad hoc duties, with our customers’ quality of life and care at the front of mind.
Administer catheter care and peg feeding
Motivate individuals and create a culture where people want to go the ‘extra mile’

We are seeking someone who will possess the following skills:

STA, SIL and NDIS Experience
In home and Community Participation coordination experience
Willing to travel for work when required
Essential social and emotional intelligence and ability to mentor and manger frontline staff and capacity to mentor, train and build teams
To go above and beyond with quality services
Essential stakeholder relationships and communication skills
ROC, Restrictive Practice, and high medical needs
Meet NDIS Compliance requirements

Qualifications & Experience

Minimum 2 years’ experience in the disability sector specifically in NDIS SIL & SDA.
Tertiary qualifications in Social Work, Disability Services, Community Services Coordination, Frontline Management or equivalent.
Advanced knowledge of the disability sector and NDIS registration groups, NDIS Plans and implementing plans.
Effective written and verbal communication.
