New permanent role for a Social Worker to join this regional health service. The location is based two hours from Canberra and four hours from Sydney. Great feedback from other candidates!
The service is a publicly funded rural hospital and community service. Be part of a large multidisciplinary team. You will look after patients in inpatient acute, subacute inpatients (rehabilitation, geriatric rehabilitation and palliative care), adult community and outpatients. Professional development and supervision supported. Subsidised accommodation available and relocation subsidy may apply. New graduates welcome to apply.
 

  • As level 1-2 Social Worker you will provide quality client-focused hospital based care to patients, their family and carers, using a holistic approach as part of a multidisciplinary team. Complete psychosocial assessments and target your interventions to meet clients needs.
  • Provide generalist counselling, assistance with navigating carer and services pathways (including MAC, NDIS pathways, centrelink, CHSP and HCP services), manage vulnerable patients needs, set goals, and assist them and their carers to achieve fair, caring and appropriate outcomes. Be part of a supportive network of health based Social Workers.
  • Be part of a large and vibrant multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers.
